The case management component is one of five components of G.R.A.C.E. Community Services. We are the Heartbeat. Our sole purpose is to assist those who have been devastated by disaster.
The case management team makes contact with potential clients by way of appointments, home visits, referrals and walk-ins. We serve the community during and after the disaster by assessing the needs of those who have been affected. We provide the necessary resources to help restore a sense of normalcy to the client’s life. We know life will never be the same, but we help them to adjust and prepare for their new life.
The Case Management Team works diligently to forge relationships with the clients to ensure that they are being provided the best available resources. Those resources should help clients to regain some sense of self-sustainability. Our goal is to empower clients so that they can get on the road to recovery. We assist individuals with the unknown. We offer clients a hand up, not a hand out. We develop a partnership with our clients. When we sit down to develop a recovery plan, all parties are involved. The plan must be realistic and is ever-changing. The only way that both parties are successful is if each person plays an integral part in the development of the plan.
Everything that we do is to empower our clients. In order for that to happen, we have to be effective listeners. Our goal is to help as many people as we can, but not with the band-aid effect. We are a long-term recovery team. We are in it for the long haul.
